Instructions

1. Download the Make a Donation by Mail form.
2. Fill out all required fields (required fields marked by an asterisk *) on Page 2 of the downloaded document.
3. Once complete, print out the form.
4. Make your check or money order payable to "JCC Education Foundation".
5. Mail the form and payment to:
